GUIDE stands for "Guiding an Improved Dementia Experience." It’s a new kind of care program designed specifically for people living with dementia and the family members who support them. This program is fully covered by Medicare.

Whether you're newly diagnosed or have been managing dementia for years, GUIDE provides coordinated, personalized support to help you live more comfortably and confidently at home.

Through GUIDE, you and your loved ones will have a dedicated care team who understands your unique needs, answers your questions, and helps you navigate each step of the dementia journey.

The program offers:

• Personalized care plans created just for you
• Regular check-ins and visits – either in-person or virtually
• Support for caregivers, including education and emotional help
• Connections to helpful community resources

We know that caring for someone with dementia can be overwhelming. It often comes with questions, worries, and moments of uncertainty, not only for the person living with dementia, but also for their family and caregivers.

Every participant in the GUIDE Program has access to a dedicated care team and a range of services tailored to your specific needs. Whether you're living with dementia or caring for someone who is, these services are here to make daily life easier and more supported.

Our GUIDE services include:

• A personalized care plan created just for you
• Ongoing virtual check-ins
• A Dementia Care Navigator to coordinate appointments and services
• Caregiver education and coaching
• Emotional and behavioral health support
• Help connecting to community resources, such as transportation, adult day programs, or home care options
• 24/7 phone support for urgent questions or concerns
  

How does GUIDE support you?

We know that caring for a loved one with dementia can feel overwhelming, isolating, and exhausting. That’s why GUIDE places a strong focus on caregiver support. You're not just part of the journey—you’re part of the care team. GUIDE helps caregivers feel seen, heard, and supported.

What can you expect?

As a caregiver enrolled in the GUIDE Program, you’ll receive:

• Your own point of contact: a Care Navigator who understands dementia and can answer your questions
• Regular check-ins to monitor how you are doing, too
• Help planning for future needs as the condition progresses
• Emotional support and stress management strategies
• Access to information and tools to improve your confidence and reduce burnout

We also connect caregivers with resources that offer relief and community, including:

• Local and virtual support groups, so you can talk with others who understand what you're going through
• Respite care options to give you a much-needed break when you need it
• Education and training to help you manage challenging behaviors, improve communication, and plan ahead

• Patients who have Traditional Medicare (not a Medicare Advantage plan)
• Diagnosed with Alzheimer’s disease or a related dementia
• Living in the community (not in a long-term care facility)
• Receiving care from our practice
  

Getting started with the GUIDE Program is simple and our team is here to walk you through every step.

2. If you’re interested in participating, we’ll schedule an appointment or phone call to begin enrollment and learn more about your needs, goals, and daily care challenges.
3. Meet with your GUIDE provider to develop a personalized care plan built around your health, lifestyle, and support system.

1. What is the cost?
The GUIDE Program is covered by Medicare. If you are eligible and enrolled in Medicare, the services provided through GUIDE are typically included under your existing coverage.

2. How is GUIDE different from regular dementia care?
GUIDE is designed specifically for individuals living with dementia and their caregivers. Unlike standard care, GUIDE provides:

• A dedicated care team that helps manage your health and supports daily living
• A personalized care plan based on your needs, values, and goals
• Support for caregivers, including education, emotional support, and planning assistance
• Ongoing check-ins and help navigating resources—often not included in typical primary care visits

3. Is GUIDE available virtually?
Yes. Most GUIDE services can be provided virtually or over the phone, including care coordination, caregiver support, and follow-up visits. This helps reduce the need to travel and ensures you can access care comfortably from home when needed.

4. Will it replace my current provider?
No. GUIDE is designed to work with your current healthcare team, not replace it. You’ll still see your primary care provider and any specialists as usual. GUIDE adds an extra layer of dementia-focused support to help you and your loved ones manage care more effectively.
