Prescription Transfer Request Form

Our team is here to help you transition your prescription needs using AdventHealth’s Rx Plus Pharmacy home delivery system.

Please fill out the form with the required prescriber and medication information.

What To Expect: Rx Plus Pharmacy will contact the prescribing provider to obtain a new 90-day prescription. This will allow you to keep your current prescription at your local pharmacy in the event an immediate need such as a vacation override comes up.

If we do not receive a response from the prescribing provider within 4 business days, we will then complete the store-to-store transfer of your current prescription from your local pharmacy.

To avoid disruption of medication therapy and to allow time for your provider and pharmacy to coordinate, it is recommended that you have at least a 2-week supply of your medication on hand prior to submitting the request.
