In 2013, we conducted our first CHNA and 2014-2016 Community Health Plan with oversight from our community CHNA committee. The Community Health Plan’s progress is continually monitored and presented annually to the hospital board. In fall 2015, we partnered with Johnson County Health and Environment to conduct a second CHNA. This two-phase research included a primary report of door-to-door surveys in the AdventHealth primary service areas of Johnson and Wyandotte Counties, and a secondary report developed for the AdventHealth geographical area that incorporates national, state and county data. This primary and secondary research, as well as hospital-specific data, served as the backdrop for the analysis to determine which priorities to address in the 2016 CHNA and 2017-2019 Community Health Plan.
Using this research, the community CHNA committee, along with the support of AdventHealth leadership, identified the following health priorities to be addressed in the 2017-2019 Community Health Plan:
- Mental health
- Access to care
- Chronic disease and risk reduction
- Poor pregnancy outcomes in low income and under-served communities
