Sometimes kids and their families need extra help to navigate multiple, ongoing medical issues that require the expertise of more than one specialist. AdventHealth for Children, formerly Florida Hospital for Children, understands how daunting multiple health conditions can be, especially when it impacts your child and even more so when one course of treatment affects another. Our Coordinated Care for Kids program is designed to ensure that children with special health care needs receive the evaluation, treatment and overall attention they need to ensure the best possible outcomes.

Coordinated Care for Kids

Special Care for Extraordinary Kids

The Coordinated Care for Kids (CCK) program is here to ensure children with special health care needs receive the best possible care, tailored to their unique needs.

This comprehensive program extends treatment outside of the hospital and is designed for patients with the following conditions.

• Chronic conditions expected to last longer than one year
• Complex conditions requiring two or more subspecialists
• Palliative conditions that are life-limiting

If you are unsure if your child meets the criteria listed above, we are happy to evaluate them to determine program eligibility.

About the National Committee for Quality Assurance (NCQA)

The NCQA Patient-Centered Medical Home program reflects the input of the American College of Physicians (ACP), American Academy of Family Physicians (AAFP), American Academy of Pediatrics (AAP) and American Osteopathic Association (AOA) and others.

It was developed to assess whether clinician practices are functioning as medical homes and recognize them for these efforts.

The NCQA Patient-Centered Medical Home standards emphasize the use of systematic, patient-centered, coordinated care that supports access, communication and patient involvement.
