Once your physician agrees that Home Care is your best route to healing, they’ll coordinate the details with your new home health care team. If you're coming from our hospital, our Transition to Home Program is aimed at making the process of leaving more comfortable and seamless. Once you're home, your team might include:

In the event of a simple health issue, skip the doctor's office and reach our physicians through our eCare app. eCare is a convenient way to talk directly with a board-certified physician or nurse practitioner through your smartphone or tablet. We'll help diagnose, treat, and prescribe medications for minor ailments, all through secure audio and video and without an appointment. Some of the common conditions we treat through eCare include:
