Free Online Access to Most Medical Records
Create an online patient account for a safe and simple way to access information from your electronic health record (EHR). For privacy and security reasons, some medical records will not be available online.
Types of Medical Records Requests
- Records for your Physician(s)

Your records can be faxed to your doctor’s office at no charge to you. You must submit the appropriate authorization form, making sure to include the physician’s name, mailing address, phone number and fax number. For faster continuity of care, AdventHealth physicians may access your medical records from their office and will not need additional permission.
A form must be completed for each physician to whom you need your medical records mailed or faxed.
- Insurance, Attorney, and Disability Requests

Records requests should be sent from your insurance company, attorney, or Disability Determination Service (DDS) and mailed to the hospital, care of Health Information Management department, or faxed to Call706-624-5006. All charges for medical records will be billed to the requestor from our copy service and must be paid before records will be shipped.
- Radiology/Imaging Films or Other Department Requests

Please contact the Radiology department for a CD copy of the images by calling Call706-602-7800 x2246 to facilitate your request. Health Information Management is only able to provide the radiology reports. You must submit the appropriate authorization form.
- Birth and Death Certificates

Birth and Death certificates are not available from AdventHealth’s Health Information Management department. These records must be ordered from the Georgia Department of Vital Records.

Paper copies of medical records may be released upon receipt of written authorization of patients over the age of 18 or a legal guardian. Once authorization is received, it may take up to 3 business days to process your request. Mental health records will only be released if selected on the authorization form or when we are given explicit consent.
If the records are needed for continued care, there is no charge to the patient.

How to obtain copies of medical records:
- A valid picture ID is required for release of medical records.
- Email or fax the completed request form to the facility’s Health Information Management (HIM) Department.
- Mail or bring the completed request form to the facility’s HIM office.
- Medical records will be mailed or emailed to the requested address.
