Located within the hospital, we offer 24-hour skilled nursing care and convenient access to the physicians, consultants and therapists to get you back to feeling your best.

Physical, occupational and speech therapy are available seven days a week, making it easier for you to meet your treatment goals and get home sooner. We’ll help you rebuild your strength, so you can to return to your independent lifestyle with renewed confidence.

Why is transitional care located in the hospital?

Our location makes it easy to deliver the care you or your loved one need to renew your strength. Doctors and specialists are available for consults, often on the same day. Additionally, occupational, speech and physical therapy are available to you seven days a week.

How long can I stay?

Our team will work closely with you to make an individualized plan that best fits your needs and helps you feel healthier as soon as possible. Our goal is to help you achieve a healthy, speedy and successful recovery through whole-person care.

Do I have to be an AdventHealth patient?

You don’t have to be a current patient in an AdventHealth hospital or facility to take advantage of the care available in our Transitional Care Unit. Call us at Call352-834-2176 to learn more.

Do you accept Medicare or Medicaid?

We accept Medicare, managed care and private insurance policies. We do not accept Medicaid.

Occupational Therapy

Occupational therapy helps you regain the skills you need to return home. Whether you’re recovering from a surgery and need extra support walking up the stairs or are relearning important skills, we’re here for you.

Physical Therapy

We understand there’s no one-size-fits-all approach to recovery. Our physical therapy team will work closely with you and your physician to create a customized treatment plan that improves your strength and flexibility. Together, we’ll get you back where you want to be: healthy, happy and at home.

Speech Therapy

Many illnesses and conditions can affect your speech. Our Speech Therapy Program, led by experienced and understanding speech pathologists, helps individuals overcome voice problems or difficulty communicating. We’ll create a plan that helps address your challenge, whether it’s an articulation problem, language deficits, oral motor skills or a swallowing disorder.

Wound Care

We provide specialized, dedicated care to help ensure your wound is healing. Our staff, which is specifically trained in wound care, is here to change bandages, clean wounds and monitor your healing process. We’ll also work with you and your family to help you feel confident and comfortable caring for your incision or wound once you return home.

24-Hour Care

We’ve designed our Transitional Care Unit to provide 24-hour care. Your team will answer questions, assess new symptoms and quickly notify doctors about any changes in your health status, regardless of the time of day. We’re here around the clock, so you and your loved ones can have peace of mind.
