Event Massage Request Form

To help us provide the highest quality care and ensure adequate scheduling, we kindly request a minimum lead time of 4-6 weeks for all massage therapy event requests. Requests submitted with less notice may not be accommodated due to staffing and scheduling constraints.

For the well-being of our therapists and to maintain service quality, the following guidelines must be observed:

Breaks:

• A 30-minute break is required if a therapist is scheduled for more than 6 hours.
• A 15-minute break is required for shifts exceeding 3 hours.
