2023 (THURSDAY) Volusia County Schools FREE Sports Physicals
About this Event
AdventHealth is the Official Healthcare Champion of Volusia County Schools and is proud to offer these FREE pre participation sports physicals and ECG screenings at the iconic Daytona International Speedway; Midway Suites.
Wednesday, April 12th, 5pm to 9pm
Thursday, April 13th, 5pm to 9pm
Friday, April 14th, 5pm to 9pm
Saturday, April 15th, 9am to 4pm
This FREE event is made available to all Volusia County Public School District student athletes, grades 6th - 12th. This includes student athletes, ROTC, band members, firefighting academy students, etc.
We will also be providing FREE ECG Screenings as part of this physical event.
Please follow the guidelines below for a successful experience:
- Be sure to download, complete and sign all required paperwork before arrival (3 forms)
- AdventHealth Consent Form
- EL2 Physical Form - front side MUST be filled out along with signature of student and parent.
- ECG Screening Consent From (WhoWePlayFor)
- Forms not signed by parent or guardian will NOT be able to receive a physical.
- All athletes must register on-line
- Walk-ins are only serviced based on availability.
