That’s why we created the AdventHealth Care Clinic, formerly AdventHealth Medical Group Transitional Care Clinic, to support your health care needs as you settle back into your home and routine. Our team of skilled and compassionate experts — which includes nurse practitioners, pharmacists, respiratory therapists and medical assistants — will support your transition so you can recover and thrive.

Our specialized services are designed to help you transition from our hospital to your home with ease. From educating you and your family on how to live with a newly diagnosed chronic condition to ensuring your medications are appropriately managed, we’re here for you.
Our program proudly serves adults in need who are over 18 years of age, underserved, uninsured or using Medicare and Medicaid.

You need expert support and guidance, especially when you’re first released from the hospital and learning to live with your new diagnosis. We’re here to help.

If you take blood thinners (also called anti-clotting medications or anti-coagulants), it’s especially important to measure the time it takes your blood to clot. With a simple blood test, your AdventHealth Care Clinic team can calculate your INR and gain important insights about whether your current medications are working effectively and decide if adjustments are needed.

- Bus passes so it’s easier for you to get to your appointments
- Food and clothing assistance from Second Harvest Food Bank of Central Florida
- Patient assistance programs like GSK inhalers, Lilly and Sanofi for insulin, and Bristol Myers Squib if you need Eliquis
With AdventHealth You’re Connected
Because we’re focused on making your transition from hospital to home as seamless as possible, our Care Clinic only offers services that help you achieve that goal. The clinic does not offer:
- Adult well visits and physicals
- Disability evaluations
- Pain management
- STD, HIV therapy and Hepatitis C treatment
- Treatment for psychiatric conditions
- Treatment with controlled medications (CII-CV)

Get Prepared Important Appointment Reminders
So you can best prepare for your AdventHealth Care Clinic appointment, we’d like to offer a few reminders. Please:
- Avoid wearing any kind of perfume, cologne or smoking cigarettes before your appointment in consideration of patients with asthma or allergies
- Bring a picture ID and a list of your current medications
- Confirm your appointment 3 business days before your visit by email, text or phone message to be sure it isn’t cancelled
If you’re a self-pay patient, you’ll also be asked to provide proof of income for the past 4 weeks (pay stubs work best) for financial qualification. Photocopies are acceptable.
