Different procedures require different corrections. For example, sometimes, after a sleeve gastrectomy, the stomach pouch becomes enlarged, so a revision to reduce the size of the sleeve to restore restriction is necessary.
A sleeve can also be converted to a gastric bypass or a SADI (Single Anastomosis Duodenal Interposition), a combination of a sleeve gastrectomy and a gastric bypass. SADI works by using both a restrictive and a malabsorptive procedure to create weight loss and resolve medical problems. It's a great procedure for people who have had a sleeve gastrectomy in the past and need a revision for weight or medical comorbidities.
While patients who previously had a gastric bypass cannot have another one, there are certain surgical options, such as making the stomach pouch smaller if it’s too big or lengthening the intestinal limbs. Another option that some patients consider is converting to a duodenal switch.

- Severe acid reflux or GERD: Certain individuals experience this more frequently after their sleeve gastrectomy and converting to gastric bypass can help relieve symptoms.
- Stretched pouch or enlarged stoma: Over time, the stomach or outlet may stretch, reducing restriction and increasing hunger or weight gain.

Weight-loss revision results are not as dramatic as the original bariatric procedure. Initial bariatric surgery changes your hormones that influence appetite and fat storage in ways that can’t be fully benefited from again when you regain weight.
If, for example, patients can lose up to 75% of their excess weight after their initial bypass procedures, a revision may enable them to lose only up to 35% of that weight. However, the weight lost may still be significant and improve a person’s whole health and quality of life.
