Answers to Frequently Asked Questions
-
Q:Question: What are the patient admission criteria?
A:Answer:To qualify, patients must:
- Need an active and ongoing therapeutic intervention of an intensive interdisciplinary rehabilitative program and can tolerate/participate in therapy at least three hours daily, five days a week, or 15 hours over seven days
- Need two or more of these therapeutic interventions: physical therapy or occupational therapy and speech therapy
- Require nursing care/supervision 24 hours per day by a nurse with training in rehabilitation care
- Have a medical and rehabilitative complexity that requires physician intervention and oversight by a rehabilitation physician (physiatrist)
- The mental and physical condition, premorbid and current, indicates the patient has potential for rehabilitation, and that functional/practical improvement can be expected in a reasonable period.
- Realistic goals for patient’s age, premorbid status, diagnosis and expected improvement, including assessment of rehabilitation potential
- Psychological and behavioral status that allows participation in the rehabilitation process, regardless of confusion, agitation or behavioral issues due to their neurological injury. Ability to consistently follow at least simple commands.
- Have a realistic discharge plan and/or family support system and must be able to meet AdventHealth financial criteria.
Patients with a spinal cord injury, regardless of etiology, level of injury and comorbidities, are assessed individually. We do not accept patients who require mechanical ventilation.
-
Q:Question: What is the discharge process like?
A:Answer:Discharge or transition planning to the next level of care begins during the preadmission and admissions phases and continues throughout the inpatient rehabilitation stay. All treatment team members, including the patient and their family or support system, share the responsibility for discharge planning.
The discharge or transition is primarily coordinated by the care manager and is based on these criteria:
- Evidence in record that the patient has achieved stated goals
- Medical complications preclude intensive rehabilitative effort
- Patient’s rehabilitation needs no longer require interdisciplinary therapy
- No further significant or measurable functional improvement is anticipated to alter discharge disposition, add practical value, improve functional capacity or impact the level of care
- Inpatient rehabilitation level of medical supervision by a physiatrist is no longer required
- No longer requires an intensive rehabilitation setting, defined as three hours of therapy five times a week or 15 hours of therapy over seven consecutive days
- The patient is not willing or able to participate in the intensive rehabilitation program for a period of more than three days
-
Q:Question: What insurance do you accept?
A:Answer:We accept Medicare, Managed Medicare, commercial insurance, Medicaid and self-pay. We work directly with patients, insurance companies and care managers to communicate coverage and associated costs based on the individual insurance plan.
