What is Home Care?

Home care provides treatment for an illness or injury that has made it difficult for you to leave your home. It helps you recover, regain your independence, and become as self-sufficient as possible in the comfort of home.

If you have a temporary to chronic or long-term health issues, home care provides qualified medical clinicians who work with you on your path to better health.

Our clinical team works with you and your doctor to evaluate your health care needs and develop your plan of care.

Physicians: Make a Referral

Skilled services we offer include: Nursing, Physical Therapy, Occupational Therapy, Speech Therapy, Social Worker and Home Health Aide.

In order to process a referral, we will need: demographics, home health order with skilled services requested, as well as the most recent visit notes.

We accept Medicare, Medicare Advantage and commercial insurance.

  • Behavioral Health Home Care

    For homebound, adult patients with depression, anxiety, bipolar disorder or any other behavioral-health issue, AdventHealth Home Care offers in-home behavioral health treatments to help foster wellness.

  • Orthopedics Program

    Getting better after surgery takes time. Our Orthopedic Program will keep you comfortable and safe during your recovery. We’ll help with your entire recovery—from managing your pain to caring for your incision sites to helping you regain mobility. And we’ll stay in contact with your doctor, ensuring your best path to healing.

  • Respiratory Program

    When you’re managing a chronic illness like chronic obstructive pulmonary disease (COPD) or a long-term illness like pneumonia, it’s important to have the right people by your side. With our Respiratory Program, you’ll breathe easier knowing you have the education, tools, and strategies necessary to find better health—and to help avoid another visit to the hospital.
