Our Transition Specialists will connect you with the health care resources needed for a successful recovery, even before you’re discharged from the hospital. We’ll coordinate the details of your transition care plan so you can focus on what matters most: healing. There are no additional fees to utilize these services, just your normal co-payments and or deductibles.
Our Transition Specialists Guide Your Path
Before you even leave our hospital, a Transition Specialist will be there to coordinate your next steps. Whether you’re transitioning back home or to a rehabilitation facility or skilled nursing facility, our team can help. Your Transition Specialist can arrange transportation for you, schedule medical appointments, communicate with your care team and more.

Physician Appointments
Follow-up care is important to your successful recovery. Your Transition Specialist will assist with scheduling follow-up appointments with your primary care physician or specialist. If you don’t have a doctor, we can help you find one.

Prescription Services
Your Transition Specialist can help find qualified programs that may assist with paying for prescriptions. In some locations, we can arrange for delivery of your medications before you leave the hospital.

Transportation Services
If needed, our team can assist you with sourcing transportation options for your hospital discharge and follow-up doctor appointments.

Meal Delivery Service
We can arrange medically prescribed home-delivered meal services if you need help transitioning to a new dietary plan, or just need a convenient way to prepare meals during your initial recovery.
Your Generosity Supports Our Care
AdventHealth Foundation West Florida has established a dedicated fund for our Care 360° Program, ensuring that our transition plans are the best they can be. When you donate, your generosity helps provide transportation for patients upon their discharge from the hospital, supplies clinical kits to patients who’ve transitioned from the hospital to home and supports other areas of need within the Care 360° Program.
