The Community Care Clinic at Altamonte is a free clinic dedicated to serving the uninsured with: asthma, COPD, diabetes, heart failure, high blood pressure, bronchitis, and pneumonia.
The clinic provides: medication cost assistance and education.
To be treated you must be: 18 years of age or older, uninsured and under 200% of the Federal poverty level.
The clinic sees patients by appointment only and is located at:

Open Monday through Thursday 8:00am to 5:00pm
Each patient receives a comprehensive medication review and assistance in enrolling in medication support programs. Pain management and treatment with controlled medications is not provided.
