Local nonprofit and community-based organizations are invited to apply for one-year grant funding as part of an initiative designed to expand community health programs and strengthen access to whole‑person care throughout Polk County.
DAVENPORT, Fla. — AdventHealth Heart of Florida and AdventHealth Lake Wales have opened the application period for the 2026 Community Impact Grant, an initiative designed to expand community health programs and strengthen access to whole‑person care throughout Polk County. Local nonprofit and community-based organizations are invited to apply for one-year grant funding aimed at addressing some of the area’s most pressing health needs.
The Community Impact Grant supports projects that deliver measurable improvements in community well‑being, with this year’s funding available in various amounts for organizations serving residents across Polk County. The program’s priorities were guided by findings from the 2025 Community Health Needs Assessments (CHNA) for AdventHealth Heart of Florida and AdventHealth Lake Wales, which identified mental health, health care access and quality and nutrition and healthy eating as the most significant needs facing the region.
With these priorities in mind, the 2026 grant cycle will focus on supporting evidence-based, programmatic initiatives that enhance quality of life, reduce barriers to care and promote healthier communities. Eligible organizations should demonstrate their ability to deliver meaningful and sustainable outcomes, collaborate with community partners, and track progress through measurable metrics.
Organizations applying for the 2026 Community Impact Grant must meet several eligibility requirements including:
- Provide programmatic services in Polk County aligned with mental health, health care access and quality, or nutrition and healthy eating, with priority given to organizations serving communities around AdventHealth Heart of Florida and AdventHealth Lake Wales.
- Demonstrate the sustainability of both the organization and the proposed program, including plans for maintaining the initiative beyond the grant period.
- Provide quarterly data to AdventHealth to track program impact and progress.
- Identify necessary partnerships or collaborations that support program success.
- Serve residents living within Polk County.
In 2025, AdventHealth Heart of Florida and AdventHealth Lake Wales invested more than $375,000 in nonprofit organizations and community partners across Polk County to strengthen essential programs that improve health, housing, behavioral wellness, and public safety, while expanding access to even more local residents.

Applications must be submitted by June 1, 2026. No late submissions will be accepted. Community organizations across Polk County are encouraged to apply and help advance health equity, improve access to care and build a stronger, healthier region for all.
