Antibiotic Prophylaxis for Peroral Cholangioscopy: Systematic Review Examines Postprocedural Cholangitis Risk

Fla. — A new study from AdventHealth Orlando physicians published in Gastrointestinal Endoscopy examines the use of antibiotic prophylaxis before peroral cholangioscopy, an advanced endoscopic procedure used to evaluate and treat biliary and pancreatic duct disorders. Although cholangioscopy use is expanding, evidence guiding periprocedural antibiotic decisions has remained limited, with recommendations often extrapolated from broader ERCP data.

The systematic review and meta-analysis examined previously published research, synthesizing findings from six studies involving 2,531 patients and 2,575 procedures. The findings suggest that antibiotic prophylaxis may be associated with a lower risk of cholangitis following peroral cholangioscopy. However, the association was borderline, sensitive to the removal of individual studies and supported by very low-certainty evidence. The authors conclude that future studies are needed to clarify which patients are most likely to benefit from prophylaxis and which antibiotic regimens are appropriate.

The AdventHealth clinicians included in the study were Tareq Alsaleh, MD; Baha Fawwaz, MD; Mohamad Khaled Almujarkesh, MD; Hamza Akram, MD; Calvin Jose, MD; Nouman Shafique, MD; Amna S. Malik, MD; Mustafa Arain, MD; and John George, MD.
