How Does It Work?
Our team of physicians and advanced practitioners specialize in providing primary care to Medicare patients. Together, we’ll develop the optimal care plan for their individual health and wellness. We make getting the care they need simple, easy, and streamlined with:
- Same- and next-day appointments
- Longer appointments, up to an hour as needed
- Board-certified physicians
- Registered nurse care manager
- Licensed clinical social worker
- On-site lab technicians
- 24/7 access to the care team by phone or online
- Pharmacist access for consultations
- Personal health coach
- Education and community events to connect patients with others who have similar interests
How Do Patients Qualify?
Well 65+ focuses on providing personalized care to patients 65-years-and-older on Original Medicare, and patients who have certain Medicare Advantage (MA) plans with:
