Access Your Medical Records

You have a right to access your medical records for personal use, and we’ve made it easy to request and receive them, either from the hospital directly or your physician.

Medical Records Before April 1, 2026
Contact Bond Clinic Medical Records at 863-293-1191, extension 3256. You can also download the Release of Protected Health Information Authorization form and fax the completed form to 863-299-0331.

Medical Records on or After April 1, 2026
Medical records from April 1, 2026, or later can be accessed via AdventHealth Medical Group.
