Our short-term inpatient rehabilitation program is designed to help patients return home safely and with confidence. These patients may include:

Steps to Home Through Coordinated Care
Our multidisciplinary rehabilitation teams help patients regain confidence, rebuild independence, and heal in body, mind, and spirit. They are committed to helping patients make meaningful progress and return home safely. During each patient’s stay, they’ll have access to the support they need, specialist consultations and patient care conferences. These conferences are an essential component for recovery, and family members are invited to attend.

To offer the best care possible, our physicians and advanced practice providers are on-site every day, closely monitoring patient progress and responding quickly to changes in condition. When needed, they coordinate promptly with medical specialists.
Our nursing team delivers attentive, compassionate care around the clock, working closely with patients to meet their daily medical and recovery needs. To best support our patients' needs, we also have respiratory therapy services available 24/7.

Our Steps to Home patients also receive onsite rehabilitation services, including:
- Physical Therapy: On-site licensed physical therapists provide therapeutic exercise, range of motion, balance, and gait training
- Occupational Therapy: On-site licensed occupational therapists assist patients with the ability to provide self-care, such as bathing, feeding, and dressing
- Speech Therapy: An on-site licensed speech therapist can assist patients with cognitive, speech, or swallowing difficulties
- Support Services and Care

- Care Management: Our Care Management team coordinates patient transitions, working with referring facilities and patient families to ensure seamless transfer into our Steps to Home program. Our team can also assist with the coordination of home services and answer questions about your continued care needs
- Pharmacy: An on-site pharmacist is available to advise physicians on medication dosages, interactions, and potential side effects. The pharmacist is also available if the patient or family member has questions about their prescription drugs
- Dietitian: Our certified dietitian is available to provide a dietary assessment to determine nutritional needs, track progress, make nutritional recommendations and provide nutrition education
