Teresa Self, DNP, APRN, FNP-C

With a strong commitment to compassionate, whole person care, Teresa Self, DNP, APRN, FNP C, chose family practice so she could care for individuals and families across every stage of life. Teresa provides comprehensive primary care services, including preventive care, well visits, chronic disease management, acute illness evaluation, and patient education. Teresa earned her Doctor of Nursing Practice with a Family Nurse Practitioner specialty from the University of Kansas’ Graduate School of Nursing. She brings a strong clinical background to her role, with many years of experience as a registered nurse in a medical surgical setting alongside her work in community based primary care. This experience allows her to manage both acute and complex chronic conditions with confidence and compassion.

Teresa values developing lasting relationships with her patients and believes trust and communication are the foundation of effective health care. Her care philosophy centers on meeting patients where they are, listening to their concerns, and empowering them with the knowledge and tools needed to actively participate in their health. She emphasizes prevention, self management, and sustainable lifestyle choices while integrating evidence based medical treatment.
