Sarah Chionuma, MSN, AGNP-C, is an adult-gerontology nurse practitioner with extensive experience in care coordination, chronic disease management and patient-centered care. Sarah earned her Master of Science in Nursing from Walden University and her Bachelor of Science in Nursing from Avila University.
