Fazlah Lakhan, MSN, APRN, FNP-C

Fazlah Lakhan, MSN, APRN, FNP-C, is a family nurse practitioner providing whole-person primary care to patients aged 12 and up. He delivers patient-centered, evidence- based care with a focus on prevention, chronic disease management and overall wellness. Fazlah brings a collaborative approach to care, working closely with patients and multidisciplinary teams to develop individualized treatment plans that support optimal health outcomes. He values open communication and partnering with patients to identify the best care options to meet their unique needs. His background includes care coordination, primary care and nursing leadership, with experience supporting patients across a variety of clinical settings. Fazlah earned a Master of Science in Nursing as a Family Nurse Practitioner and a Bachelor of Science in Nursing from Chamberlain University. He is nationally certified as a family nurse practitioner and holds an advanced practice registered nurse license

Outside of work, Fazlah enjoys spending time outdoors, exploring Florida’s wildlife and nature, going on cruises and attending car shows.

Fazlah works collaboratively with patients and a multidisciplinary care team to accomplish treatment goals with care and compassion. He believes the best outcomes are achieved through strong partnerships, thoughtful communication and personalized care.
