Building Health in Chicago’s Western Suburbs
Our partnership with the University of Chicago Medicine brings leading-edge medicine with whole-person care to Chicago’s western suburbs. Our four local UChicago Medicine AdventHealth hospitals strive to live out the calling of helping others find wholeness, even beyond our walls.
Our community benefit programs look to meet the needs of our friends and neighbors in Cook, DuPage and Will counties, caring for their physical, mental and spiritual health. We want our presence to help make our communities healthier.
We are committed to growing deep roots into our local communities.
The Power of a Nationwide Network of Care Combined With Local Focus
-
Community Programs
Our community health programs are driven by our desire to extend whole-person healing outside our buildings. Because caring for our patients’ whole health in body, mind and spirit impacts the communities around them, we desire to shape the areas we serve with care to embrace the whole community.
-
Community Health Needs Assessments and Plans
Every three years, we conduct community health needs assessments (CHNAs) to determine how we can help improve the lives of people who live near our hospitals. These assessments guide our plans and programming.

How Health Plans Drive Community Care
The health of people living in the same community can be very different due to many influencing factors. Our Community Health Plans, built on a three-year rolling cycle, outline how UChicago Medicine AdventHealth Bolingbrook, UChicago Medicine AdventHealth GlenOaks, UChicago Medicine AdventHealth Hinsdale and UChicago Medicine AdventHealth La Grange have identified priorities in our greater Chicagoland community. These priorities include healthcare access and quality, mental health, chronic disease management and food insecurity.
- Healthcare Access and Quality
-
Some of the programs we conduct to meet this goal include:
- Medical mission clinic
- Medication therapy management clinic
- Collaboration and financial support for DuPage Health Coalition
- Collaboration and financial support for Pillars Community Health
- Collaboration with VNA Health Care
- Mental Health
-
As a healthcare provider, we want to treat more than just our community’s physical health. Being well mentally impacts every other marker of health.
Some of our mental health initiatives in Chicago include:
- Mental Health First Aid training in collaboration with the National Alliance on Mental Health (NAMI) Metro Suburban and DuPage, and the DuPage Health Coalition
- Will Grundy and DuPage Health Coalition
- Chronic Disease Management
-
When a person’s chronic disease is treated and controlled, it can improve their quality of life, which is what we want for all our neighbors.
We work toward this goal by offering health education and screenings throughout the year.
- Food Insecurity
-
We’re addressing the area’s food insecurity in several ways, including mobile food pantries in collaboration with Northern Illinois Food Bank, micro pantries and financial support for the Islamic Circle of North America (ICNA) Relief Pantry.

Community Health Needs Assessments and Plans
Every three years, we conduct a community health needs assessment (CHNA) to learn the needs of our surrounding area. This assessment helps us develop a three-year plan to meet the needs of residents in our community.
