• Find the right AdventHealth Provider Network primary care provider (PCP) and other specialists as needed.
• Schedule or request appointments with AdventHealth Provider Network doctors.
• Coordinate your health care needs across the AdventHealth Provider Network with your PCP, other doctors and any necessary screening or testing appointments.

AdventHealth On Demand offers a whole tugboat-load of easy-to-access care options to ensure you are well-covered and well-cared for as an AdventHealth CastCare℠ member. You’ll receive expert, award-winning AdventHealth services on your schedule, from primary care doctor visits to urgent care needs, delivered, well, everywhere — even at home.

Wouldn’t it be great to have a sidekick on your health care journey? Now you can. Our expert nurse care managers work with you and your primary care provider to keep you on the best path to a happy, whole life. Our care managers will:

• Help you develop a personalized care plan to make managing your health easier
• Support you in managing your chronic conditions, such as diabetes or asthma, by connecting you with tools, education and resources to help you improve your health
• Work with a care manager to assist you in setting goals to better manage your medications, lose weight, stop smoking or develop an exercise plan
