Full-time: Coverage equal to your annual salary (rounded to the nearest $1,000)

Part-time: You can buy coverage equal to your annual salary, based on your hourly rate and scheduled hours.

We'll guide you through the next steps and assist with initiating your claim.

  • What is Evidence of Insurability (EOI)?

    Some life insurance elections require Evidence of Insurability (EOI) — a health review completed by the insurance carrier before additional coverage can be approved.

    EOI is typically required if you:

    • Elect coverage above the Guaranteed Issue amount.
    • Enroll after your initial eligibility period.
    • Increase your supplemental life insurance beyond the amount allowed without EOI.

    If EOI is required, we'll let you know. You'll receive instructions to complete a health questionnaire, and the insurance carrier will review your application. Your additional coverage will not take effect until it has been approved.
