- Final Paycheck & PTO

Your Final Paycheck
You’ll receive your regular earnings, less any applicable deductions. Your final paycheck will be paid through your usual method — direct deposit or pay card — on the standard pay cycle, or as required by state law.
PTO Payout
If you are a PTO-eligible team member and have completed one year of service, your PTO accrual will stop on your termination date. Any unused PTO and Legacy PTO balances will be paid out as follows:
- If you resign voluntarily: You’ll receive 90% of your unused PTO and 100% of your Legacy PTO balance.
- If you retire: You’ll receive 100% of your unused PTO and 100% of your Legacy PTO balance.
Payout will be processed less applicable taxes and withholdings, in accordance with AdventHealth policy and state law. Please allow one to two pay periods to receive your payout.
- Educational Assistance

We are proud to support team members in their educational goals through AdventHealth's Educational Assistance program.
If you leave AdventHealth within 24 months of receiving Educational Assistance, a portion of the assistance may need to be repaid:
- Within 12 months of receiving assistance: 100% repayment
- 13–24 months after receiving assistance: 50% repayment
If repayment is required, you will receive a notice outlining the amount owed and available repayment options. Consistent with applicable state and federal laws, AdventHealth may deduct repayment amounts from your final paycheck, including eligible hours worked and any PTO payout.
If a remaining balance exists after any applicable paycheck deductions, repayment is expected within 30 days of receiving the notice. Balances that remain unpaid after that time may be referred to a third-party collection agency.
Please note that team members with an outstanding Educational Assistance balance are not eligible for rehire until the balance has been paid in full.
- Health Coverage After Separation

Medical Coverage
Your medical coverage ends on your separation date. But don’t worry — you can continue coverage for up to 12 months through Continuation of Coverage (COC), including prescription coverage.
- Watch for your COC packet from Inspira Financial within 2 weeks. You can also contact Inspira Financial at Call888-678-7835 with questions about your COC packet.
Important: You’ll need to enroll within 30 days of the date on your COC offer letter.
Medicare Enrollment Reminder
If you or a covered dependent delayed enrolling in Medicare Part B, you may need proof of prior Group Medical coverage to avoid a late‑enrollment penalty.
Please contact HR Shared Services to request documentation of your AdventHealth medical coverage for Social Security purposes.
Timing matters — be sure to request this documentation as soon as possible after your separation.
Dental & Vision
Dental and vision coverage end on your separation date and cannot be continued.
- Flexible Spending Accounts (FSA)

You can still submit claims for eligible expenses you incurred before your coverage ended. All claims must be submitted to Optum Financial within 90 days of your coverage end date. Expenses after your last day of eligibility aren’t covered.

Your HSA is yours to keep — even after you leave AdventHealth! Contributions stop on your separation date, but you can:
- Keep the account (fees may apply)
- Move or withdraw your funds
No new contributions are allowed after separation. For details on options, fees or transferring your balance, contact Optum Financial: myoptumfinancial.com/AH or Call844-680-5661.
- Disability & Life Insurance

Short-Term Disability (STD)
Ends immediately.
Important — If you have an open or upcoming disability claim, contact the carrier right away to avoid delays.
Long-Term Disability (LTD)
You may be eligible to convert your LTD coverage to an individual policy if:
- You’ve been insured for at least 12 months
- You were actively at work before separation
- You apply for conversion
Deadline — Submit your application and payment within 62 days of your employment end date.
Important — If you have an open or upcoming disability claim, contact the carrier right away to avoid delays.
Life Insurance
- May also be converted to an individual policy within 62 days.

Supplemental Plans and Legal, Pet and Identity Protection

- Adventist Healthcare Retirement Plan (AHRP)

You can request a distribution once AHRP receives your separation information from AdventHealth (usually about two weeks after processing).
Accounts with balances of $1,000 or less may be automatically distributed per plan rules.

- Seventh-day Adventist Hospital Retirement Plan

If you worked at an Adventist-owned facility before January 1, 1992, you may qualify for monthly annuity benefits under this inactive pension plan.
Benefits start only when you reach the plan’s qualifying age.
Deadline: Call at least 3 months before your 65th birthday to begin the process.
