Start here — your LOA checklist

Talk with your manager
Share your anticipated last day at work and how long you expect to be out. For intermittent leave, coordinate pre‑planned time away and follow your department call‑out process when an absence is unforeseen.

Update your contact info in Workday
Go to Profile → Contact → Edit to update your personal email, mailing address, all phone numbers and emergency contact. We use this if we need to reach you during leave.

Apply for leave with New York Life (NYL)
- Option 1: Online at myNYLGBS.com
- Option 2: Call 800‑852‑0682 to start your claim, then finish setup on myNYLGBS.com
Your manager will be notified after you submit the request and again when a decision is made.

Watch for NYL updates and respond quickly
All letters and claim updates post to myNYLGBS.com and are mailed to your address in Workday. Read every message and complete any requests so your leave keeps moving.
Types of Leave

Family and Medical Leave Act (FMLA)
Job-protected time for family and health needs.
FMLA provides up to 12 weeks of job-protected leave during a rolling 12-month period for your own serious health condition, caring for a spouse, child or parent or bonding after the birth, adoption or foster placement of a child. You're eligible after 12 months of employment and 1,250 hours worked in the past year. Leave may be continuous or intermittent with required documentation. PTO is used first, and benefits may continue as long as premiums are paid.

Medical Leave of Absence
Support for your recovery when FMLA doesn’t apply.
If you need time away for your own serious health condition, Medical Leave is available from your first day of employment. This leave provides up to 12 weeks of job-protected time, requires medical certification and must be taken continuously. PTO applies first, and benefits can continue as long as premiums are paid.

Personal Leave
Flexibility for life’s important needs.
Personal Leave offers up to up to 3 months of continuous leave, within a rolling 12-month period, for approved personal reasons, such as caring for a family member who doesn’t qualify under FMLA or pursuing unique opportunities. This leave is available upon hire, requires leader approval and is not job-protected, meaning reinstatement isn’t guaranteed.

Military Leave
Serving those who serve.
Military Leave includes Qualifying Exigency Leave — up to 12 weeks for urgent family needs during deployment — and Military Caregiver Leave, which provides up to 26 weeks to care for a servicemember. These leaves apply to a spouse, child, parent or next of kin and are job-protected under USERRA and FMLA.

Educational Leave
Time away for professional growth.
Team members can take up to 3 months of unpaid time, within a rolling 12-month period, for approved educational purposes. This leave requires leader approval, must be continuous, and is not job-protected.

GME Paid Leave
Paid time off for Resident Physicians.
Resident Physicians in years 1–7 of an accredited program may take up to six weeks of paid leave at 100% of base wages for health needs, family care or bonding with a newborn or adoption. This leave runs concurrently with other applicable leaves and may be continuous or intermittent.
Your Guide to LOA
- Short‑term disability (STD) — if your leave is for your own health

If your leave includes an STD claim:
- Give NYL permission to contact your provider and make sure your provider has permission to share records with NYL
- Ask your provider to send notes, test results, treatment plans, restrictions and an estimated return date to NYL quickly
- Confirm your actual first day of leave with NYL when it starts; processing begins after you confirm
- STD approval and LOA approval are separate decisions; your NYL letters will show the status of each
- Set up direct deposit in myNYLGBS.com if approved; STD payments come from NYL and will not include your normal benefit deductions
- How do I plan intermittent time away?

Coordinate pre‑planned days with your manager and follow call‑out procedures for unforeseen absences.
- When do I report intermittent absences?

Report each intermittent absence to NYL and your leader within one business day, or as soon as reasonably practicable. If you do not report promptly, that time may not be job‑protected.

- Keep watching for messages from NYL and respond to any requests
- If your paid time off runs out, contact HR Shared Services to arrange payment for your portion of voluntary benefits
How to reach HR Shared Services (HRSS)

- Or call 844‑843‑6363, Option 1, to speak with a Benefits Specialist
- If your leave is not approved

- If you are absent for your own health condition, contact HRSS at 844‑843‑6363 and ask to speak with a Return-to-Work (RTW) Specialist
- If your absence is for any other reason, contact your manager for guidance on returning to work
- Returning from a medical leave (your own health)

- Confirm with your provider
As your end date approaches, talk with your provider. If you need an extension or plan to return earlier, inform NYL and your manager. - Upload your clearance in Workday
Go to Menu → Absence → Request Return from Leave of Absence- Enter your first day back
- Enter your last day of absence
- Add comments if helpful
- Upload Return‑to‑Work Clearance (Fitness for Duty)
- Submit your request.
- Watch for approval
HRSS processes clearances within three business days if there are no restrictions. If there are restrictions, an ADA Specialist will review before authorization to return.- Badge access is generally restored within 24 hours of the approved end date
- If you do not see confirmation in three business days, call HRSS
- Confirm with your provider
- Returning from a non‑medical leave

- Update NYL and your manager if your leave end date changes
- Submit a return request in Workday
Menu → Absence → Request Return from Leave of Absence- First day back
- Last day of absence
- Comments
- Submit your request. Your leader will receive a task to review and approve.
- Badge access is generally restored within 24 hours of the approved end date in NYL
- Returning from military service (USERRA) of 30+ days

- Update NYL and your manager if your leave end date changes
- Submit your USERRA reemployment documents in Workday
Menu → Absence → Request Return from Leave of Absence- First day back
- Last day of absence
- Comments
- Upload Reemployment Letter, DD214 or other supporting documentation
HRSS processes USERRA reemployment within three business days
- Badge access is generally restored within 24 hours of the approved end date in NYL
Tip: If you see an error in Workday when you submit, double‑check your dates, confirm you uploaded medical clearance if needed and make sure your profile shows on leave. If your profile shows active, contact HRSS.
